Waiver and Release of Liability
SANTA CRUZ TAEKWONDO ACADEMY · A PROGRAMME OF WORLD TAEKWONDO MANDEVILLE · REGISTERED UNDER WORLD TAEKWONDO JAMAICA
This Waiver and Release of Liability (the "Agreement") is made by the registrant — and, where the student is under eighteen (18), by the student's parent or legal guardian — in connection with participation in all training sessions, grading examinations, competitions, events and activities organised by or affiliated with Santa Cruz Taekwondo Academy, a programme of World Taekwondo Mandeville ("WTM"), delivered at the Social Development Commission Office, Centre Road, Santa Cruz, St. Elizabeth, Jamaica, under the instruction of Master Kenroy Clarke.
1. Acknowledgement of risk
Taekwondo is a contact martial art and physical activity. I understand it carries inherent risks of injury including bruises, sprains, strains, fractures, dislocations, concussion and, in rare cases, serious or permanent injury or death. I have considered these risks and voluntarily accept them for myself and/or for the minor student I am registering.
2. Release and indemnity
In return for being allowed to take part, I release, waive and discharge, and agree not to sue, Santa Cruz Taekwondo Academy, World Taekwondo Mandeville, World Taekwondo Jamaica, Master Kenroy Clarke, their instructors, officers and volunteers, and the Social Development Commission as owner and operator of the training venue (together the "Released Parties") from all liability, claims, demands and causes of action arising from participation in any programme activity, whether caused by the negligence of the Released Parties or otherwise. I further agree to indemnify the Released Parties against any loss, cost or expense they incur as a result of my or the student's participation.
3. Medical authorisation and health declaration
If a medical emergency occurs and I cannot be reached in time, I authorise the Released Parties to arrange and consent to emergency medical treatment for the student, and I accept financial responsibility for any medical costs. I confirm the student is in good health and fit to train, and that I have disclosed every relevant medical condition, allergy and medication on this form.
4. Code of conduct
Students, parents and guardians are expected to show courtesy, discipline, respect and sportsmanship on and off the mat, and to treat the Social Development Commission premises with care. The programme may suspend or dismiss a student for misconduct without refund. Attendance obligations are set out in clause 10.
5. Fees and payment
Fees are: a one-time registration fee of J$1,000; monthly tuition of J$8,500 per student, due at the start of each month; and a dobok (uniform) at J$7,000 regardless of size. Thirty (30) days' written notice will be given of any fee change. Tuition paid is non-refundable. Unpaid fees may result in suspension of training until the balance is settled.
6. Grading and rank
Rank advancement follows the Kukkiwon standard through World Taekwondo Mandeville and World Taekwondo Jamaica. Students are entered for grading when the instructor judges them ready on syllabus, attendance and attitude. Rank is earned at the individual's pace and is never guaranteed within a fixed time.
7. Photography and media
The programme photographs and records students at training, gradings and competitions for use on its website, social media and promotional material. Consent to this is optional and is given by the separate checkbox below; it can be withdrawn at any time in writing to worldtaekwondomandeville@gmail.com.
8. Privacy
Registration information is used only to administer the programme and communicate with families. It is held in the World Taekwondo Mandeville student roster and is not sold, rented or shared with third parties without consent, except where the law requires.
9. Communication
I agree to keep an accurate phone number and email address on file and to respond within a reasonable time to official communications, including fee notices, schedule changes, grading announcements and emergency alerts. The programme communicates mainly by WhatsApp, email and announcements in class. Failure to respond to critical communications may lead to a temporary suspension of training until the matter is resolved.
10. Participation and attendance
Regular attendance is essential to progress and safety. I will give advance notice of planned absences where possible, and I understand that extended or unannounced absences may affect eligibility for grading, team selection and competition. Students are expected to arrive prepared, in the correct dobok, with a respectful attitude.
Attendance policy. A student who misses five (5) consecutive scheduled classes without prior notice or a satisfactory explanation is automatically removed from the programme. Re-enrolment requires a new registration and the registration fee. A second removal for the same reason may result in permanent dismissal from World Taekwondo Mandeville programmes, subject to written appeal to the Head Instructor.
If circumstances — medical, financial or personal — affect attendance, speak with Master Clarke first. Solutions are always explored before any formal action.
11. Governing law
This Agreement is governed by the laws of Jamaica, and any dispute under it is subject to the exclusive jurisdiction of the courts of Jamaica.
By signing below you confirm that you have read, understood and voluntarily agree to every term of this Agreement. If the student is under 18, this form must be completed and signed by a parent or legal guardian.
